MEDICAL HISTORY

Dental treatment although it might seem trivial, it still is a medical act therefore it can be affected or interact with other medical conditions or medications.
At your first visit, you should inform the dentist if you suffer from certain diseases that might affect your dental treatment. Don't hesitate to mention it, as irrelevant as it may seem. The only thing you might need is a simple filling or a cleaning but your dentist should be aware of your complete medical history, in case your treatment needs to be modified accordingly.

You can download the medical history form we use and send it prefilled via email or print it and bring it on your first visit
